Step 1: Initial Consultation

  • Schedule a free, initial consultation using the calendar app.

  • During the initial consultation, you should provide background information about your child and share your advocacy goals. We can provide preliminary feedback and answer any questions you have about the advocacy process.

Step 2: Intake

  • After your initial consultation, please complete the Contract for Services and Intake Form.

  • You should also submit records relevant to supporting your advocacy goals (such as assessment reports and IEPs).

  • After reviewing these records, Martinez Advocacy will schedule an intake meeting with you.

Step 3: Complete Additional Forms

  • Once you are a client, this Authorization Form grants Martinez Advoacy permission to communicate with your child’s school, doctors, and any additional providers that support them.
